Maintenance records are required for truck and trailer. Everything you do from day 1. Truck and trailer are separate records like Unit 1 and Trailer Unit 1 or however you want to indentify them.
There are two records for each one and they are separate. One is the Preventive maint and the other is the repairs, etc. Use the owners manual PM record. Best way is to make a spreadsheet. It will have to say what the pm to be performed is and mileage, date, and who did the pm and comments column for whatever comments need to be made.
The repair record just lists repairs that are performed like replacing tires, replacing injectors, headlights, or whatever.
PM record is lube, oil change, air filters, etc. Whatever is on the factory owner record.
Each record (spreadsheet) has to list the tire size and VIN number and vehicle number (unit #) company name, truck model, make etc.. Each record will have the type record it is in the heading.
The records are kept in the office but I always kept a copy with me. The records include all receipts for service and repairs.
You also need a drivers record for each driver.
I used to have a link to a website that had the info about every record required but I can't find it now.
If you will send me an email so that I can have your address I will send you a copy of my maint record. You can modify it as you desire for your use if you want.

This is on the FMCSA website. There is lots of info there. Get familiar with it and use the search function to find most of what you need.
A separate record for each driver is required even if there is only one driver. You need to send off for a copy of your state driving record every year to update the record including one when you start your business. The link below tells you what has to be in the record.
This is a great electronic drivers log. I used it and loved it. Kept me straight. Won't let you cheat. You will need a printer in your truck to print the log sheets out for the DOT checks.

You will need to save the files to your harddrive and use Microsoft Excel to open them.
