The following form is only for those that are running more than one truck. If you have one truck and are the only driver, then it's not required. The TAB order is off a bit. Still working on fixing it.
All forms and information posted here may be outdated, or apply to your operation. They are presented here, only as a guide line to help you in setting up your own forms:

This is a good step TG. Getting setup is a hard step when you don't which way to start.

Good info TG, usable by even us Vetran Truckers. LOL. Really liked the FSC article that went with the chart. Puts things in historical prospective, I was not trucking in 2000 and was surprized by the jump in prices in 2000. Must have really hurt guys back then, too.

The fuel surcharge article says what I said on that other website. Fuel surcharge has nothing to do with fuel mileage. That chart is based on price only and that is what I tried to point out.
I got banned because of particular know it all smart mouth that had no clue what he was talking about. That's communism for you, be right and step on toes and they close you down...just like Putin did in Russia when the media revealed his 24 yr old lover. They shutdown the newpaper. Oh, this is politics so I may get banned!!!
